Please request a leave packet two months prior to the due date. We will email you a packet as well as send a hard copy packet to your home address. This packet will contain forms for you and the health care provider to complete as well as information on how to submit a short-term disability claim and apply for Parental Paid Leave, if eligible.
Once you and the health care provider have completed the forms, please upload them to our Leave Form Submittal Portal. This is the preferred way to ensure your paperwork is processed in a timely manner.
